About consent

You are an adult when you are 18 years old or more.

When you are an adult, you can make your own decisions about the National Disability Insurance Scheme (NDIS).

This means we need to ask for your consent.

When you give your consent, you say it is okay for someone to do something.

How you can give us your consent

You can give consent by filling out an NDIS consent forms.

You can also give us your consent in a:

  • plan meeting – when we talk about what supports you will get
  • check-in meeting – when we make sure your plan is working well for you.

When you give consent, we will make sure you understand what you agree to.

We will make sure you give consent because you want to.

Not because you feel like you have to.

We will make sure you choose:

  • how long the consent will last
  • what exactly you are giving consent for.

We will check these things every time you give us your consent.

We will record every time you give us your consent in our computer system.

You can take away your consent whenever you want to.

What we need your consent for

We will ask for your consent before we:

  • share your information with anyone
  • ask someone for information about you.

This includes asking for your consent before we share your NDIS plan with anyone.

Your NDIS plan explains how the NDIS will support you.

We will ask for your consent before we check your identity documents.

Identity documents:

  • show who you are
  • have your personal information on them.

For example, a driver’s licence or a Medicare card.

We will also ask for your consent to check your:

  • age
  • address.

Giving your consent to providers

Providers support people who take part in the NDIS by delivering a service.

Providers need your consent to see:

  • your personal information
  • parts of your NDIS plan.

You need to tell us if you want to give consent to a provider.

You can choose which providers you give your consent to.

And you can choose the parts of your plan you give each provider consent to see.

Giving somebody consent to help you

You can choose to give another person consent to help you with the NDIS.

For example, you can give them consent to:

  • talk to the NDIA on your behalf
  • get letters with information about you
  • do things on your behalf, like ask us to change your plan.

They can support you to make your own decisions about the NDIS.

They cannot make decisions for you.
