When your plan ends

Your NDIS plan explains how the NDIS will support you.

Every time your plan ends, we will do a plan reassessment with you.

When we do a plan reassessment, we check to see if the supports in your plan still work well for you

We check plans in different ways.

This is because people need different things.

We will work with you to make sure your new plan has the right supports for you.

Before your plan reassessment

4 months before your plan reassessment, your my NDIS contact will talk to you about the plan you have now.

Your my NDIS contact is a support person who you have a lot of contact with.

Your my NDIS contact will look at:

  • the information we have about you
  • how you use your funding.

Funding is the money from your plan that pays for the supports you need.

Your my NDIS contact will also look at the community supports and services you use.

They will also look at: 

  • any new information you share about your life
  • reports from your providers.

Providers support people who take part in the NDIS by delivering a service.

Making your new plan like the plan you have now

We can make your new NDIS plan like the plan you have now.

We can do this if: 

  • you tell your my NDIS contact that you are happy with the plan you have 
  • you have the right supports you need
  • we think your plan supports your goals

You can still change small parts of your plan.

We can make your new plan last as long as the plan you have now.

You can also manage your new plan the same way.

You can find your new plan on the my NDIS participant portal and app when it is ready.

You can start using your new plan straight away.

You will not need to have a meeting for us to approve your plan.

When we approve your plan, we agree to everything in it.

Making your new plan different to the plan you have now

Your new plan might need to be different from the plan you have now.

We will work with you to check what should change in your plan.

Your my NDIS contact will talk to you before your plan reassessment.

You can tell them about:

  • what parts of your life have changed 
  • the new or different supports you need.

Your my NDIS contact can support you to get ready for your plan reassessment.

They can explain: 

  • what information you need to have 
  • how you can find this information
  • how your plan reassessment will work.

If you have a support coordinator, they can help you with this too.

A support coordinator is someone who helps you plan and use your supports.

A psychosocial recovery coach can also help you get ready for your plan assessment.

A psychosocial recovery coach is someone who helps people with psychosocial disability.

A psychosocial recovery coach: 

  • knows about mental health
  • helps you be more independent
  • helps you make the most of your plan

We will start with the plan you have now. Then we will think about what needs to change.

After we approve your plan, you can find and use it on the my NDIS participant portal and app.
