There are 2 ways to change your plan.

You can ask to change your plan any time.

About changing your plan

You might need different supports if parts of your life change.

If this happens, we will work with you to change your plan.

We will ask you:

  • what you want to change
  • why you want to change your plan.

We will also help you find other ways you can use the plan you have now.

You might be able to use your supports in a different way.

We will also explain:

  • the different ways we can change your plan
  • what information we need.

There are 2 ways we can change your plan.

1. Plan variation

A plan variation is when we change small parts of your plan

2. Plan reassessment

When we do a plan reassessment, we check to see if the supports in your plan still work well for you

How we make decisions about changing your plan

We use information to make a decision about changing your plan.

We will look at:

  • the information you give us 
  • what you want to change.

We will make sure the supports in your plan will still work well together.

We will also make sure we can give you funding for the supports you want to change.

Funding is the money from your plan that pays for the supports you need.

We will tell you if we need more information.

Then we will decide whether your plan needs to:

  • change
  • stay the same

If we decide to change your plan, we will work with you to do it.

We will change your plan within 21 days.

If we decide not to change your plan, we will tell you why.

We will also ask if you want a meeting to help you use your plan.

We call this a plan implementation meeting.

We will also tell you what you can do if you do not agree with our decision.

Plan variation

We can do a plan variation if you only need to change small parts of your plan.

We can do a plan variation to: 

  • fix something that is wrong in your plan
  • change the date of your plan reassessment
  • change how you manage your plan

A plan variation can also give you funding for more supports if something changes in your life.

For example, if you:

  • have an accident
  • cannot live in your home anymore

We might give you this funding while we make a new plan for you.

Plan reassessment

We do a plan reassessment when you need a new plan.

This can happen when: 

  • your plan is ending
  • you need to change your plan in a big way.

You might need to change your plan because you need different supports now.

You might also ask for something new that changes the other supports you need.

Your my NDIS contact will help you get ready for your plan reassessment.

They will explain:

  • what information you need
  • how to get the right information 
  • how we do a plan reassessment.

We will look at all the information about your plan.

We will share your new plan after the plan reassessment.
